Errors made by health care personnel with the COVID vaccine came in at No. 5 out of the top 10 drug errors for all of 2020.
The Institute for Safe Medication Practices compiles a Top 10 list every year and even though the COVID vaccines barely slid into 2020 in December, they still managed to come out at No. 5. Errors included mistakes in storage and temperature conditions, mixing and diluting the vaccines and confusing the vaccines with similarly-named products, to name a few.
In one instance patients received injections of a treatment drug for COVID, rather than the vaccine. In another report, vaccines were administered to patients younger than the age indicated for use of the vaccine. “Hazards resulting from infusion pumps being positioned outside of COVID-19 patients’ rooms” came in at No. 4.
